Prospective Health Executive Summary:

Since 2003, Physician WebLink has worked closely with the employees and dependents of Duke University and Healthcare System to design and implement the Duke Prospective Health Program. The overall goal of Prospective Health is to provide an individualized guide to healthier living for all participants and their families. It is expected that the improved quality of life for employees and their dependents will also result in lower overall medical costs for Duke University. The focus of the program for 2004 is on cardiovascular disease, hypertension, diabetes, and preventive health services. Using a multi-faceted approach Prospective Health gives individuals the ability to take an active role in improving their overall health and well being and to meet their personal health goals. This includes the use of an internet based Health Risk Assessment (HRA) to personalize their plan. Interventions combine both self-directed and nurse/physician interactions to monitor and assist the individual navigate to a successful outcome. Participants focus on specific areas of life to improve overall health and well being. Program participants can range from people in tip top physical shape to those living with a chronic illness.
